Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV was known to use his/her wheelchair as a walker and was unstable on his/her feet. According to an investigation, AV experienced approximately six falls between November 15, 2024, and March 1, 2025, resulting pain, skin tears, head injuries, and fractured ribs causing severe pain and unreasonable discomfort. The facility failed to provide appropriate services according to Alleged Victim’s needs, relating to care plans and lack of interventions and instructions, to mitigate the risk of injury due to AV's falls, which is a violation of resident rights, is neglect of care which constitutes abuse.
