Findings: The Alleged Victim (AV) is a known fall risk. Interventions in place were to ensure that AV had non skid socks on while in bed and to be reminded to use his/her assistive device while ambulating. On or about February 16, 2025, AV was found on the floor in the staff break room. AV was not wearing socks, nor did he/she have his/her assistive device. AV suffered a skin tear from the unwitnessed fall. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00801 $250.00 fine assessed
