Findings: On or about March 6, 2025, the Alleged Victim (AV) was found lying on his/her right side, next to his/her bed. AV suffered a hematoma to the head and discoloration to his/her right arm. AV was on hospice, so hospice was called and it was determined that AV had a broken arm and was given pain medication. AV had a bed alarm on his/her bed, however, it did not notify staff of the fall, as the alarm was not working. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00405 $1125.00 fine assessed
