Findings: On or about December 2024, it was noted that the Alleged Victim (AV) had some discolored skin on his/her backside. On or about December 20, 2024, a temporary service plan was put into place regarding AV's discolored skin and instructed care staff to report any further concerns to the Nurse. Staff were to encourage AV to come out of his/her room and to apply lotion to AV's backside. On or about March 10, 2025, it was discovered that AV had multiple skin discolorations and it appeared no care had been provided to AV to ensure his/her skin condition was being monitored. AV was taken to his/her physician, who determined it was likely AV's skin discolorations could have developed into worsening pressure injuries if not treated. The facility's failure to provide proper skin care is a violation of resident rights, is considered neglect of care and constitutes abuse.
