Findings: According to the documentation, the facility failed to properly care plan appropriate interventions for the Alleged Victim’s (AV) fall history. On or about February 25, 2025, the AV experienced a significant change of condition which resulted in higher care needs and service plan update. Between March 06, 2025, March 11, 2025, the AV experienced four (4) falls with injury, including bruising, pain, and unreasonable discomfort. There was no documented evidence the facility implemented interventions to prevent the AV from experiencing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00821 $375.00 fine assessed
