Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, on or about October 26, 2024, the food pushcart was under the control of Alleged Perpetrator 2 (AP2) when it made contact with AV. AV reported that AV then fell and experienced pain. According to the Department’s investigation, the facility (AP1) failed to provide AV with a safe environment, which is a violation of resident rights, is neglect which constitutes abuse. The allegation against AP2 was investigated and determined to be not substantiated.
