Findings: The Alleged Victim (AV) is known to leave the facility unsafely. On or about March 11, 2025, AV was visibly upset and wanting to leave the facility. Staff were trying to redirect AV, however, AV was sitting on a bench in the independent living area, and asked staff to leave him/her alone. Staff left and went back to check on AV and he/she was missing. Approximately 45 minutes later, a community member called the facility stating they found AV on the shoulder of a 5 lane highway, attempting to cross to road. AV was brought back to the facility. Respondent failed to ensure resident safety, placing AV at risk for harm. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
