Findings: The Alleged Victim (AV) and Witness 1 (W1) rely on the facility for care and have a history of two previous physical altercations with each other. On or about March 14, 2025, AV was attempting to leave the room they share with W1, when W1 attempted to stop them by grabbing the door and AVs wrist. AV screamed for help and during the altercation AV sustained a skin tear on their wrist. AV also expressed being nervous to go back into their room. The facility failed to implement effective interventions and / or appropriately care plan for W1 and AV's history of physical altercations, and failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
