Findings: The Alleged Victim (AV) relies on the facility for their care, including medication administration. According to an investigation, on or about March 16, 2025, AV had been given the wrong dosage of anxiety medication, resulting in a fall that caused pain in AV's rib. The facility failed to administer medication as ordered and ensure that Alleged Perpetrator 2 (AP2) was properly trained, which is a violation of resident rights, is neglect of care which constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) neglected AV was investigated and determined to be not substantiated.
