Findings: The Alleged Victim (AV) was care planned to not leave the facility without supervision, as AV is a known wander risk. On or about March 17, 2025, AV left the facility unattended and was gone for approximately 4 hours without anyone knowing where he/she was. AV was not noticed to be missing until approximately 8:00 PM during the med pass. AV was found and returned to the facility unharmed, however, AV was placed at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00437 $250.00 fine assessed
