Findings: According to the documentation, the facility failed to follow the care plan and assist with bathing for the Alleged Victim (AV). from approximately January 01, 2025, through March 28, 2025, the facility staff failed to assist with bathing about zero (0) times. The AV had suffered a fall in the shower by themselves and did not feel safe to shower alone as they were prone to falls. The failure to assist the AV with bathing is a violation of resident rights, is considered neglect of care and constitutes abuse.
