Findings: The Alleged Victim (AV) relies on the facility for his/her care. On or about February 12, 2025, AV received discharge instructions to keep AV’s split dry and non-weight bearing. According to an investigation, on or about February 17, 2025, AV was unable to get the hard cast because AV’s split had been completely saturated, and AV’s leg was swollen. AV was also noted to have difficulty remaining non-weight bearing during transfers. As a result, AV had another soft split put on. On or about February 25, 2024, AV was no longer able to have the case placed as the fracture had changed and needed surgery. The facility failed to provide medical treatment as ordered, which is a violation of resident rights, is neglect of care and constitutes abuse.
