Findings: According to the documentation, the facility failed to follow the care plan for putting on the Alleged Victim’s (AV) brace on their arm. On or about March 25, 2025, the AV had bruising on their left arm. An investigation by the facility determined the facility did not follow the care plan and put the brace on too tight, causing bruising, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
