Findings: Alleged Victim (AV) relies on the facility for his/her care, and is care planned to have toileting assistance. On or about March 26, 2025, Alleged Perpetrator #2(AP2) was assisting AV to the restroom and left AV unattended. AV fell off the toilet and sustained bruises on AV’s left collar and right/hip butt area and complained of pain in the right leg when moved. AP2 failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the care plan was being followed, which is a violation of Oregon Administrative Rules.
