Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, on 3/27/25 AV was able to exit from their secured unit and go into an unsecured area of the facility. The facility failed to ensure that the exit door was functioning properly, allowing AV to gain access to a stairwell, which is a violation of resident rights and considered neglect of care which constitutes abuse.
