Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls in March and April 2025. On or about April 4, 2025, AV suffered a fall in his/her room and was yelling for help. AV was found to have a distended abdomen after his/her fall. AV was sent to the hospital and diagnosed with a fractured rib. The facility's failure to implement meaningful interventions to prevent AV from falling is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00803 $1125.00 fine assessed
