Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) were assisting AV up from the floor. AP2 grabbed AV's arm roughly, which caused AV discomfort. AP3 witnessed AP2's treatment of AV and did not report this timely to the facility management as required. AV was later sent to the hospital and diagnosed with a fracture of the arm. The facility failed to provide a safe environment for AV, which is a violation of Oregon Administrative Rules. The actions of AP2 and AP3 are a violation of resident rights, are considered neglect of care and constitutes abuse.
