Findings: Witness #3 (W3) has known sexualized behaviors and had been on 1:1 care at the facility for these actions. The Alleged Victim (AV) was found on more than one occasion with W3 in his/her room, with W3 being naked and or trying to undress AV. The facility's failure to ensure AV's safety from W3 is a violation of resident rights, is considered neglect of care and constitutes abuse.
