Findings: The Alleged Victim (AV) was a known fall risk with a history of falls. According to an investigation, AV experienced multiple falls between January 6, 2025, and April 8, 2025, which resulted in various abrasions and injuries, including a head injury that required three staples. The facility failed to implement appropriate interventions to mitigate the risk of injury due to AV's increase in falls, which is a violation of resident rights, is neglect of care which constitutes abuse.
