Findings: Alleged Victim (AV) relies on the facility for assistance with mobility, which includes two-person transfers and use of a gait belt. According to an investigation, on or about April 13, 2025, Alleged Perpetrator 3 (AP3) transferred AV without a gait belt resulting in a skin injury. It was discovered that AP3 was new to the facility and not trained how to do a two-person transfer or how to use a gait belt. The facility failed to follow provide a safe environment, which is a violation of resident rights, is neglect of care which constitutes abuse. The allegation that AP3 neglected AV was investigated and determined to be not substantiated.
