Findings: The Alleged Victim (AV) relies on the facility for his/her care and has a history of falls. According to an investigation, on or about, April 22, 2025, AV experienced an unwitnessed fall, resulting a skin tear on back of their head. The facility failed to appropriately plan care and/or implement interventions to mitigate the risk of injury due to AV's increase in falls, which is a violation of resident rights, is neglect of care which constitutes abuse.
