Findings: The Alleged Victim (AV) relies on the facility for their care and has a history of falls. AV is care planned for using a walker and known to be unable to use the call system. According to an investigation, on or about April 26, 2025, AV experienced an unwitnessed fall, resulting in a head injury. AV was not using their walker and did not ask for help or use the call system. The facility failed to appropriately plan care, related to appropriate interventions to mitigate the risk of injury to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
