Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is a known fall risk. Between January 26th through March 26, 2025, AV had 14 falls. As a result of the falls, AV suffered skin tears/abrasions to his/her arms and back. The temporary care plan provided after the falls did not implement progressive measure to prevent AV from falling. The facility failed to care plan and mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
