Findings: The Alleged Victim (AV) relies on the facility for their care and has a history of falls. According to an investigation, AV experienced approximately two falls between February 28, 2025, and March 8, 2025, and experienced another fall on or about April 27, 2025, resulting in a lump on the right side of AV’s forehead and right arm pain. The facility failed to appropriately plan care, related to appropriate interventions to mitigate the risk of injury to AV due to AV's increase in falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
