Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, on or about May 1, 2025, AV playfully poked at Witness 1 (W1) and W1 struck AV in the face. W1 has been the involved in multiple resident to resident altercations. The facility failed to implement interventions for the staff to help prevent W1's behaviors and did not provide a safe environment for AV, which is neglect of care and constitutes abuse.
