Findings: Alleged Victim (AV) relies on the facility for his/her safety. On or about May 3, 2025, AV eloped from the locked facility sometime after 5pm. AV was found 3.4 to 4 miles away from the facility. As a result of the elopement, AV fell and sustained a sprained ankle. The facility failed to provide a safe environment which resulted in AV eloping and sustaining a sprained ankle, which resulted in harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-01253 $500.00 fine assessed
