Findings: Witness #1 (W1) is known to have physical aggressions against other residents. W1 does not like having a roommate, and will do anything to have them removed. On or about April 28, 2025, W1 pushed AV down onto his/her bed and was holding him/her there. Staff intervened and separated the two. Respondent failed to ensure AV safety by placing AV as a roommate with W1, placing AV at risk for harm. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
Sanction: RCFCP26-00046 $250.00 fine assessed
