Findings: The Alleged Victim (AV) is known to have physical aggressions against other residents. AV does not like having a roommate and will do anything to have them removed. On or about April 28, 2025, AV pushed WItness #1 (W1) down onto his/her bed and was holding him/her there. Staff intervened and separated the two. Respondent failed to ensure AV safety by placing W1 as a roommate with AV, placing AV at risk for harm. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
