Findings: The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of falls. According to an investigation, on or about April 21, 20245 AV fell. There was no documented evidence of monitoring AV after the fall. On or about April 28, 2025, redness was observed by a care staff on AV's hip and on April 30, 2025, the hip showed a bruise with swelling. On or about April 30, 2025, AV's forearm was observed with redness, warmth and swelling. Progress notes on April 21, 2025, showed interventions for staff to increase monitoring at night and assist AV with sitting, but these interventions were not on the service plan or communicated to staff. The facility did not implement any known changes to AV's care plan following the fall and monitor for injury. The facility failed to properly plan care for AV, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
