Findings: The Alleged Victim (AV) relies on the facility for care and services. According to an investigation, AV suffered a fall on or about May 5, 2025 and the facility did not put interventions in place or update the care plan with interventions for AV falling. AV continued to show signs of pain as noted by facility staff. AV suffered another fall on May 14, 2025, which resulted in a bilateral hip fracture. The facility failed to implement safety measures to help mitigate AV from falling, which is a violation of resident's rights, is considered neglect and constitutes abuse.
