Findings: The Alleged Victim (AV) and Witness (1) rely on the facility for his/her care. W1 has a history of altercations and was care planned to be kept separated from other residents when showing behaviors. According to an investigation, on or about May 11, 2025, AV and W1 were involved in an altercation resulting in scratches and nail marks to AV's forearm. W1 had been displaying behaviors in recent days prior to the altercation on May 11, 2025. The facility failed to properly address W1's behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
