Findings: The Alleged Victim (AV) has a history of eloping the facility and exit seeking behaviors. On or about May 9, 2025, AV was found outside of the memory care facility, on the assisted living side. It is unknown how AV left the memory care; however, AV has at times jiggled and push on door handles. Respondent failed to ensure AV's safety by not supervising AV appropriately to avoid an elopement. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
