Findings: According to the documentation, the facility failed to provide a safe medication administration system for the Alleged Victim (AV) by administering a discontinued medication for approximately 2.5 months. On or about February 16, 2025, AV’s primary care provider discontinued a medication which potentially had negative side effects from another medication. The facility failed to update the medication administration record for the AV to discontinue this medication. The medication was continued to be administered to the AV until approximately May 09, 2025. The failure to update the medication administration record for the AV put the AV at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
