Findings: The Alleged Victim (AV) relies on the facility for their care and to provide a safe environment. The AV required assistance with transfers, but the facility care plan was not updated and has AV as independent with transfers. AV also has a history of self-transferring and this was not reflected in AV’s care plan. According to an investigation, on or about April 9, 2025, AV fell while attempting to self-transfer and sustained a hip fracture from the fall. The facility’s failure to update AV’s care plan and implement interventions for AV self-transferring are a violation of resident rights and are neglect of care which constitutes abuse.
Sanction: ALFCP25-00331 $1500.00 fine assessed
