Findings: The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of falls. According to an investigation, on or about May 1, 2025, AV fell out of a wheelchair while being assisted by staff, resulting in injury to AV's eye and head. AV's service planned interventions were for staff to give AV an item to hold onto when pushing AV in the wheelchair so AV would not grab onto furniture or try to stand. Staff were aware AV would sometimes put their feet down while being pushed in the wheelchair. The facility did not develop or implement any known changes to AV's care plan to address the behavior of AV putting their feet down. The facility failed to properly plan care for AV, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
