Findings: The Alleged Victim (AV) relies on the facility for care and services. AV requires assistance with mobility, transfers, and toileting. According to the investigation, AV was sent out of the facility to an appointment without the assistance of a staff member from the facility and was left outside in the cold weather for 20 minutes prior to the service provider's office being opened. AV was not dressed appropriately for the weather and had an episode of incontinence while at the service provider's office. The facility failed to provide the care and services needed to keep AV safe, which is a violation of resident's rights and is neglect of care and constitutes abuse.
