Findings: The Alleged Victim (AV) relies on the facility to provide care and services. According to an investigation, AV's care plan states that AV has a history of falls and interventions were implemented to help mitigate AV from falling. One of the interventions was that AV was to have a bed alarm in place and working when AV is in bed. On or about May 12, 2025, AV was found on the floor of AV's room and had suffered an unwitnessed fall. The facility failed to follow the care plan, and the bed alarm was not in place at the time AV was found on the floor, which is a violation of resident rights, is neglect of care which constitutes abuse.
