Findings: Witness #2 (W2) has known sexualized behaviors and had been on 1:1 care at the facility for these actions. On or about May 13, 2025, W2 was kissing AV and placing his/her hand up AV's shirt. The facility had noted that W2 was back at baseline and no longer needed 1:1 oversight to ensure that W2 would not make sexual advances towards other residents. The facility's failure to ensure AV's safety from W2 is a violation of resident rights, is considered neglect of care and constitutes abuse.
