Findings: Witness #2 (W2) has known sexualized behaviors and had been on 1:1 care at the facility for these actions. On or about May 13, 2025, W2 was kissing AV while laying on top of AV. The facility had noted that W2 was back at baseline and no longer needed 1:1 oversight to ensure that W2 would not make sexual advances towards other residents. The facility's failure to ensure AV's safety from W2 is a violation of resident rights, is considered neglect of care and constitutes abuse.
