Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is not receiving help from staff with dressing and transfers on and off the commode. AV needs assistance with pulling pants up and down but staff refuse to do this for AV. The facility has identified that AV needs assistance with transfer but has failed to update AV’s service plan and provide interventions for transfer. An unknown Alleged Perpetrator 2 (AP2) pressed into AV’s leg using the leg for leverage to stand up while providing cares. AP2 failed to provide a safe environment to AV, which is neglect of care and constitutes abuse. The facility failed to care plan around AV’s needs, which is a violation of resident rights, is neglect of care and constitutes abuse.
