Findings: The Alleged Victim (AV) relies on the facility for their care. W1 is known to be anxious or agitated when shopping and can get agitated in group settings. On or about, May 16, 2025, AV and W1 had an altercation during a free store event at the facility, resulting in bruises on AV’s forearms. The facility failed to provide a safe environment, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
