Findings: The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about May 24, 2025, AV fell while taking a shower. AV tried to get help by using their call pendant and also a pull cord in the room, but neither were working. AV then maneuvered to the hallway to call for help. After approximately thirty minutes, AV called their relative who got ahold of facility staff to assist the AV. The facility failed to provide a safe environment, when the call system was not working order, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: ALFCP25-01003 $250.00 fine assessed
