Findings: Alleged Victim (AV) and Witness 1 (W1) rely on the facility for their safety and care needs. W1 has a history of aggressive behaviors and resident-to-resident altercations in the dining room. On or about May 25, 2025, AV was pulling on the tablecloth, and W1 did not like that. W1 hit AV's hand. The facility failed to adjust the service plan for W1 leading to continued aggressive behavior, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
