Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, AV has a history of trying to elope from the facility. On or about May 28, 2025, AV was able to push through a door and exit the facility. When AV was outside, AV attempted to go into traffic multiple times. The facility had to call law enforcement to try and keep AV out of traffic, and AV was taken by law enforcement to the hospital. The facility failed to provide a safe environment for AV, which is a violation of resident's rights and is neglect of care which constitutes abuse.
