Findings: Witness 1 (W1) and Alleged Victim (AV) have history of behaviors and resident-to-resident altercations. On or about May 26, 2025, AV and W1 had a resident-to-resident altercation in the dining room. Interventions put into place, keep AV and W1 away from each other in the dining room. On or about May 31, 2025, AV and W1 were in the dining room, when W1 kicked AV and rammed W1's wheelchair into AV. Av pushed W1 back and W1 reached up and grabbed AV's face causing AV to have bloody nose. The facility failed to have a safe environment, oversight of AV and W1 and failed to follow interventions in place for W1 and AV, resulting in a resident-to-resident altercation, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
