Findings: On or about May 15, 2025, the Alleged Victim (AV) was in the common area and had expressed to staff that he/she was wanting to go to bed. AV requires a one person assist with ambulation while using his/her wheelchair. Staff failed to assist AV to his/her room to help him/her into bed, leading to AV being found on the floor in his/her room, just outside his/her bathroom. AV was not harmed, however, AV was placed at risk for harm. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00828 $1000.00 fine assessed
