Findings: The Alleged Victim (AV) relies on the facility to provide services and care. According to an investigation on or about May 24, 2025, AV suffered a fall at the facility and hit their head. The staff did not put in place and monitoring of AV post the fall until May 27, 2025, putting AV at risk of harm due to the lack of care and oversight of AV after the fall. The facility failed to ensure that AV was safe post fall, which is a violation of resident rights, is neglect of care which constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) failed to ensure AV's safety was investigated and determined to be not substantiated.
