Findings: The Alleged Victim (AV) relies on the facility for their care and is identified as a fall risk. According to an investigation, the AV experienced approximately six falls between the timeframe of April 4, 2024, and June 4, 20245, resulting skin tears, head injuries, and repeated unreasonable discomfort. The facility failed to provide appropriate services according to the AV needs, relating to care plans and lack of appropriate interventions and instructions, to mitigate the risk of injury due to AV's increase in falls, which is a violation of resident rights, is neglect of care, and constitutes abuse.
