Findings: The Alleged Victim (AV) relies on the facility for care and to provide a safe environment. According to an investigation, on or about June 8, 2025, Witness 1 (W1) and AV were hitting each other and has to be separated by a staff member. At the time of the incident, staff were to be monitoring AV and W1 when they were together in common areas of the facility, and that was not being done. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect which constitutes abuse.
