Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on or about, June 9, 2025, AV was discovered unresponsive and sweating in the courtyard. The ambient temperature was determined to be around 98 degrees, and AV had been in the courtyard between thirty minutes to an hour, resulting in a sunburn and risk of harm. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
